TEAM REGISTRATION

The season will begin September 17, 2024 and end February 27, 2025. We ask that all parents review the following policies below as well as have all forms filled out and turned in by the first practice. They can either be printed/scanned/emailed to louisianahockey@gmail.com or brought to the first practice at Planet Ice on 9/17/24.

 

*NOTE: The last form link will be a Sponsorship form that is not mandatory but a way for you to donate to the LAHA organization as well as receive a discount on your team registration cost.
